2. What information do we collect about you?

We will collect, store and use the following categories of personal information about you:

  • Personal contact details such as name, title, address, postcode, telephone number and email address;
  • Information about your personal circumstances that may be relevant to the service you have requested, e.g. in making a subject access request or complaint.

We may collect proof of identity, proof of address, photograph and signature in the course of verifying your identity for subject access. We may also collect any other information that you supply us with to help us locate the information you are requesting which may include details of other family members, or if submitting on behalf of another individual we will capture both parties’ details.

You will be informed of any other data we collect, that is not listed above, orally or through email at the time of collection of the data.

If you fail to provide certain information, we may not be able to provide the service you have requested.
